HOW DO I MAKE AN APPOINTMENT?

 

During business hours you can call us at 813-964-5901. You can also book online by clicking here. We do request a $50.00 deposit, which will be applied to your visit.

 

WHAT DO I NEED FOR MY INITIAL APPOINTMENT?

 

  • Submit the patient forms online in advance of your appointment
  • Government issued ID
  • Any medical records supporting your reason for cannabis usage

 

HOW LONG IS THE INITIAL APPOINTMENT?

 

The initial appointment can be 45 - 60 minutes.

 

WHAT IS DONE ON THE INITIAL APPOINTMENT?

 

All required paperwork is completed. You will be seen by the physician who will review your medical records, and perform the required medical evaluation. The physician will also review with you the basics regarding cannabis and your cannabinoid system, cannabis components (THC, CBD, Terpenes), methods of delivery and what would be best for your issue(s). We will also assist you in the application process so that when you leave us, everything will have been done and all you need to do is wait for your approval from the State.

 

BESIDES OBTAINING MY CANNABIS CERTIFICATION, IS THERE ANYTHING ELSE YOU CAN DO FOR ME?

 

Yes. This is a Functional Medicine practice. That means our goal is to find out the root cause of an issue and repair it, or make it better. A lot of our patients seeking medical marijuana obtain a more comprehensive evaluation and testing that was not offered or available to them previously. One major issue we see is Lyme Disease. This is also called, "The Great Imitator", and can mimic Rheumatoid Arthritis, MS, ALS, Autoimmune Diseases, Fibromyalgia, Chronic Fatigue Syndrome and even Alzheimer's and Parkinson's Diseases.

 

If you are looking for a more comprehensive approach to your issue(s) let us know as you will need to fill out a more comprehensive history form. You will also be charged for an Initial Functional Medicine evaluation, which will include your Cannabis Certification. When you become a cannabis patient, should you decide to come back for a more comprehensive Functional Medicine evaluation at a later date, we offer reduced pricing for this evaluation.

 

WHAT ARE THE COSTS AND HOW OFTEN DO I NEED TO BE SEEN?

 

Our Initial Cannabis Certification is only $165.00. If you want a full Functional Medicine evaluation, the cost is $300.00 for the initial evaluation. should you decide to come back for a more comprehensive Functional Medicine evaluation at a later date, we offer reduced pricing for this evaluation. Per State regulations certifications last 210 days (7months) and then you must be seen by the qualifying physician again face-to-face for a re-certification. Our Recertification Evaluation is also only $165.00.

 

CAN I OBTAIN MY MEDICAL MARIJUANA CARD BY A VIRTUAL VISIT OR OVER THE PHONE?

 

No. The State of Florida mandates that you be seen by a physician initially and every 210 days (7months) for recertification. This must be a face-to-face visit with the physician.

 

WHAT IF I NEED TO RESCHEDULE?

 

You can call us at 813-964-5901 to reschedule, or if you signed up online, you can go to your patient portal and reschedule. The $50.00 deposit will transfer to your rescheduled appointment.

 

WHAT FORMS OF CANNABIS ARE AVAILABLE?

 

There are various routes of administration. Inhalation (vaping), Smoking (flower), Oral (tinctures, liquids, capsules,, concentrates), Sublingual (under the tongue liquids and lozenges), topical creams and suppositories. Edibles should be available in 2020.

 

HOW DO I OBTAIN MY MEDICAL MARIJUANA CARD?

 

Florida requires that patients obtain a certification from a qualifying physician during a face to face medical evaluation. A physician must be qualified by the State of Florida in order to issue you a certificate to receive a card. Qualifying physicians must have completed training specific to medicinal marijuana. After obtaining a certification (and being entered into the registry), a patient must submit an application to the Office of Medical Marijuana Use to obtain an ID card. Once a patient receives their temporary (via email) or permanent ID card and a recommendation order from their physician they may purchase medication at a licensed dispensary.

 

We will assist you with everything that is required, before you leave our facility, so that the only thing you need to do is wait for your approval.

 

HOW DO I QUALIFY FOR A MEDICAL MARIJUANA CARD?

 

The State of Florida has identified 10 qualifying conditions. They also have another category called Medical Conditions of the Same Kind or Class that allow a qualified physician to approve you if you do not meet any of the 10 qualifying conditions.

 

DO I HAVE TO BE A RESIDENT OF FLORIDA TO OBTAIN A MEDICAL MARIJUANA CARD?

 

You need to be either a permanent Florida resident, or a seasonal resident. Either way you must have proof of residency. For permanent Florida residents, a valid Florida driver license or Florida identification card is needed. For seasonal residents that do not posses a valid Florida driver license or identification card you must submit two of the following documents:

 

  •  A deed, mortgage, monthly mortgage statement, mortgage payment booklet or residential rental or lease agreement.
  • One proof of residential address from the seasonal resident’s parent, step-parent, legal guardian or other person with whom the seasonal resident resides and a statement from the person with whom the seasonal resident resides stating that the seasonal resident does reside with him or her.
  • A utility hookup or work order dated within 60 days before registration in the medical use registry.
  • A utility bill, not more than 2 months old.
  • Mail from a financial institution, including checking, savings, or investment account statements, not more than 2 months old.
  • Mail from a federal, state, county, or municipal government agency, not more than 2 months old.

 

For all forms of residency, the name and address on the document(s) provided must match the name and address provided in the application.

 

IS THERE A FEE TO THE STATE OF FLORIDA TO OBTAIN MY MEDICAL MARIJUANA CARD?

 

The State of Florida charges a $75.00 fee for your initial application and card. This card is valid for one year. The yearly renewal fee is also $75.00 and can be submitted online within 45 days of your card expiring.

 

WHAT ARE THE 10 QUALIFYING CONDITIONS?

 

  • Cancer
  • Epilepsy
  • HIV
  • AIDS
  • Glaucoma
  • Post-traumatic Stress Disorder
  • Amyotrophic Lateral Sclerosis (ALS)
  • Crohn's Disease
  • Multiple Sclerosis (MS)
  • Parkinson's Disease

 

WHAT IS A CONDITION OF THE SAME KIND OR CLASS?

 

A qualified physician can certify you if you are suffering from a condition that has similar symptoms as one of the 10 qualifying conditions listed above. Some examples would be: Irritable Bowel Syndrome (IBS), which is similar to Crohn's Disease. Anxiety and/or depression are similar to Post-traumatic Stress Disorder. Chronic pain and or muscle spasm can be similar to ALS or MS.

 

HOW LONG DOES IT TAKE TO GET APPROVED BY THE STATE?

 

It is typically taking between 5 - 10 working days for you to get approved by the State of Florida. You will first receive approval online. The card will come at a later date.

 

WHEN CAN I PURCHASE PRODUCTS?

 

As soon as you obtain online approval you are able to purchase products from a dispensary. You do not need your marijuana card, but you must have a form of picture ID.

 

WHERE DO I OBTAIN CANNABIS PRODUCTS?

CAN SOMEONE ELSE OBTAIN THE MEDICAL CANNABIS FOR ME?

 

Only if this person is your designated Caregiver. Caregivers must be added to the Medical Marijuana Registry and submit a complete application and yearly fee to obtain an identification card.

 

CAN I USE MY MEDICAL MARIJUANA CARD FROM ANOTHER STATE?

 

No. The State of Florida does not allow reciprocity from other States.
